Update OpenStack contrib to use per-cluster inventory layout

Supports Kubespray workspace with multiple OpenStack-deployed k8s
clusters.

* Create sample inventory directory for template
* Moved broken `group_vars` symlink to sample directory
* Created sample cluster Terraform file
* Updated documentation
